I simply copied Ala's Apply and filled in my infos since I couldnt find an application form.
Hope thats ok.

Age: 18
Where do you live ?: In Germany
How many Languages can you speak ?: German, English, Turkish
Ingame Informations:
Nickname: fr0zeN
PastNicknames: J@cOB, ProoF, uNknOwn ( just for a few days. Explanaition follows.)
USGN: Pr0oF/19092
Experience: 3 Years. Played my noob phase without an USGN Account, so it doesnt fit to my USGN Number.
Pastclans: Some M4A1 Clans around the PCS Servers.
WhydoyouwanttobeaVIP ?: Sometimes there are just not enough Admins around and when there is another speedhacker on the server, there is noone to ban him.
Why wewould take you as VIP ?: Im skilled, active and mature. I can handel the rights.
As the title shows I would like to apply for being VIP in the FWS Shotgun Server.
My current nick is fr0zen. Right now I'm clanless but in near future I will open a clan together with ala's. We will probably introduce ourselfs then in your "Introduction" area.
I'm pretty skilled and I'm mature enough to handle VIP rights.
I know some of the ppl here(old snake, Brainless, ala's ...).
Some of you might know me as J@cOB(used this nick years ago), ProoF(my "M4A1" nick) or uNknOwn. I used this nick a few days until I realized there is someone called "unknown" here. So I changed my name to fr0zeN and I intend to stay with this nick.

Vips don't actually kick, it's the job of mods and admins. Anyway good luck
